The Anthem Foundation is now accepting applications for the second cycle of their 2017 grant program, which provides funding for community health programs.
Programs must address issues related to five specific categories:
#1 Healthy Hearts: Programs that minimize controllable cardiovascular diseases and strokes, including smoking, obesity, hypertension and stressful and sedentary lifestyles.
#2 Healthy Cancer Prevention: Programs that implement life-saving cancer prevention and early-detection programs.
#3 Healthy Maternal Practices: Programs that encourage and facilitate first trimester prenatal care and help at-risk mothers commit to behaviors that reduce the numbers of low-birthweight babies.
#4 Healthy Diabetes Prevention: Programs that educate communities on the health risk of diabetes and how to prevent it.
#5 Healthy Active Lifestyles: Programs that raise awareness, educate and encourage new behaviors, resulting in healthy active lifestyles.
The foundation prioritizes applicants that can provide measurable results of success and growth in their community health programs, such as a fitness initiative that can point to lowered BMI for a target population. Proposals will be evaluated based on three questions:
- What results will be achieved?
- Will this organization deliver on its proposed commitments?
- Is this grant the best possible use of the Foundation’s resources?
Awards generally range from $25,000 to $125,000, according to Inside Philanthropy, though the foundation has awarded community health programs grants up to $1 million.
Grants are available to tax-exempt public charities classified as 501c3 status in the states of Arizona, California, Colorado, Connecticut, Florida, Georgia, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Missouri, Nevada, New Hampshire, New Jersey, New York, Ohio, Tennessee, Texas, Virginia, Washington, West Virginia and Wisconsin.
Applications are due Sept. 1.
